Northern Manhattan Community Assessment

CONTEXT		GOALS
Population Ethnically/racially diverse adults with serious mental illness (SMI) residing in Northern Manhattan. Specialty mental and physical health clinics, community and faith based organizations.	Unmet Needs Racial, ethnic, and linguistic disparities in health and healthcare in people with SMI. Lack of culturally competent care Fragmentation and lack of coordination of care.	Enhance the cultural competence and quality of healthcare for racially and ethnically underserved people with SMI. Manualize best practices for culturally & linguistically competent service integration.

Multi-level Key Informants

	Assess mental & physical health programs and delivery of culturally and linguistically competentcare.	Map Community Resources (e.g., physical, mental health services, community and faith based organizations, recreational activities).	Community Resource Directory. Reports and Presentations to inform participating clinics, stakeholders, policy makers, researchers, patients and their families. Develop services and policy recommendations Manualize best practices for culturally & linguistically competent service integration.
	Identify strengths, capacity and needs for meeting the needs of our population of interest.	Sites Select 5-6 mental health clinics serving patients with SMI in Northern Manhattan.
