Product Recalls
As part of its ongoing hazard surveillance program, the Institute maintains a system for reviewing the recall of products which are defective or in some manner deemed unsafe for continued use.
  • The Institute contracts with the National Recall Alert Center NRAC National Recall Alert Center to provide ongoing surveillance and written notification of all known recalls involving bio-medical products.
  • Subscribes to a web-based FDA notification system regarding bio-medical and related devices.

The Institute also maintains surveillance of food product recalls via Nation Food Safety Database, and Fire Safety related recalls via U.S. Fire Administration.

All notices are received at the office of the Environment of Care Committee Chairman. Relevant bio-medical notices are promptly forwarded to the Clinical Deputy Director for further review and follow-up. The Clinical Deputy will either determine that the item is not in use here or order it's removal from our inventory.

Recalls of food products - related to food allergies or contaminated products - are promptly forwarded to the Nutrition Services Administrator for further review and follow-up. The Nutrition Services Administrator will either determine that the item is not in use here, or order it's removal from our inventory. (The Nutrition Services Administrator and other Nutrition Services staff maintain their own close surveillance over the quality and safety of incoming food products. Any item of processed food found to be problematic is promptly brought to the attention of the supplier. As appropriate, notice is also sent to OMH Nutrition Services to alert other facilities as to the defective product.)   

The Clinical Deputy returns all notices to the Environment of Care Committee Chairman to indicate outcome of his review and further actions taken or recommended.

The results are read into the minutes of the Environment of Care Committee at its next meeting and the matter is open to further discussion as appropriate.

Copies of all recall notices are maintained at the office of the Environment of Care Committee Chairman for a minimum of one year.
