Accidents

The administration takes appropriate measures to reduce the frequency and severity of accidents within it buildings, on its grounds and in its vehicles. Through its Committee on the Environment of Care, it works toward this goal by:

Encouraging all staff, patients, and visitors to report accidents or other untoward events in which they may be involved or which they witness while on our premises. Encouraging all to suggest ways to improve safety and reduce likelihood of future accidents.

Reviewing actual accidents to determine direct causes, contributing factors, trends.

  • Inhalation of noxious fumes or gases
  • Automobile crashes

Reviewing external sources for information re accidents & prevention.

Taking remedial action to prevent recurrences

Removing unsafe physical conditions

Repairing or replacing defective or unsafe equipment

Purchasing additional materials or equipment

Changing procedures

Training and retraining staff

Reviewing post-intervention experiences.

Reporting on results of program.

The Chief Safety & Security Officer will report on all unintended events directly involving a person or persons, which caused, or might have caused occupational illness or injury to staff or injury to patients or visitors, or damage to property including equipment and vehicles within the period preceding the current meeting.

He will, as appropriate, indicate the outcome of his department's investigation into the nature, cause(s), effect(s) of each event. He will indicate follow-up and remedial action taken or recommended. The Committee will be alerted as to trends (by type of accident, job titles, locations or times of events, etc), and discuss possible adjustments to policy and/or practice as needed for future improvement of conditions. Such information, along with recommendations for improvement will be shared with appropriate departments and over-site groups.

An analysis of accidents is part of the EOC Program's annual report on all aspects of the Environment of Care Program.
