Agreement made as of this 22nd Day of September 2000 by and between New York State Psychiatric Institute and R-Wave Biomedical Services, Inc., having its principal office and place of business at 173 Newtown Road, Plainview, N.Y. 11803, hereby amends contract C000096 between the aforesaid parties as stated below:

By extending the term of the contract for twenty-four months beginning 10/01/2000 through 09/30/2002.

By increasing the cost per year.

By replacing the listing of medical equipment with the listing of the present equipment as per Attachment I.

It is understood and agreed by and between the parties that all other provisions of contract number C000096, unless otherwise amended, shall remain in full force and effect.
